Pain and Endorphins

Preventing Maternal Exhaustion and Dehydration: for Moms and Birthworkers


Preventing, Detecting and Treating Maternal Exhaustion and Dehydration

Exhaustion occurs as the body’s ability to meet the stress diminishes and finally gives out.  A major cause of clinical exhaustion is the loss of potassium ions.  As cells lose potassium, their functioning is increasingly impaired until they finally start for to die.  Unless this process is rapidly reversed, vital organs will stop functioning.  Another component of exhaustion involves the depletion of adrenal glucocorticoids which is the result of a sudden fall in blood glucose and a lack of nourishment for the cells.  It is of vital importance to conserve the laboring mothers energy and taking necessary steps to keep her well nourished on physical and psychological levels.   Encourage the mother to eat, drink, and rest when possible throughout labor.    Your alertness for possible signs of exhaustion and dehydration should be heightened as labor lasts longer and longer.
Signs of impending exhaustion: 
·         Ketonuria of +2 or more may be the first sign of impending clinical exhaustion and dehydration
·         High specific gravity values is an early sign of dehydration
·         Dry mucous membranes
·         Rising pulse above 100 (note normal range)
·         A rising temperature of above 101F (which may result in fetal tachycardia)
Checking for dehydration:
The tilt test is one way of determining the degree of dehydration.     Measure the pulse and blood pressure with the woman lying flat on her back.  Then have her it sit upright for one minute with her legs dangling off the edge of the bed and reassess her vitals.  Her vitals should remain approximately the same for each reading.   You can try a third time if necessary.  If the person is dehydrated, exertion from arising results in temporary vasoconstriction which raises the pulse rate.
Moderate dehydration is a 10 bpm increase in pulse. 
Severe dehydration if the pulse rises as much as 30bmp.
When exhaustion and dehydration are present, you must act quickly to turn it around.

Coconut water is the liquid endosperm inside young coconuts.   Endosperm is the tissue produced in the seeds of most flowering plants around the time of fertilization. It surrounds the embryo and provides nutrition in the form of starch, though it can also contain oils and protein. This makes endosperm an important source of nutrition in human diet. For example, wheat endosperm is ground into flour for bread, while barley endosperm is the main source for beer production. Other examples for edible endosperm are coconut "meat", popcorn and banana.
Coconut water is a natural isotonic beverage which has the same electrolytic balance as our blood.  It is identical to human blood plasma which makes it a universal donor.  Plasma makes up 55% of our human blood therefore by drinking it we get an instant blood transfusion.   Coconut water is also a blood purifier.
Anne Frye’s electrolyte balancing recipe


1 tsp miso
1tsp lemon juice
1 tsp salt
1 tsp honey
1 tsp calcium (liquid or powdered)
2 cups water - warm to dilute ingredients.   Have her sips slowly but surely!


Salt/Sodium  has the most “grounding” descending activity of any substance used as food.  In the Ayurvedic  tradition, the active quality of salt was emphasized as markedly strengthening one’s energy.  It can be used to a person “down to earth”.

Potassium is a mineral that naturally balances the metabolic action of sodium.  When there is deficient potassium relative to sodium, neuromuscular function decreases – the body becomes weak, and reflexes become poor.   High concentrations of potassium are found in every vegetable, all types of potatoes (with skins), soy products, millet and other grains, and legumes; also in bananas and most other fruits.  In terms of labor, it is unlikely that you will be able to get her to eat much.  There food are important throughout pregnancy for prevention.    If there is need you could make banana/soymilk smoothy, add some honey.

Honey contains minerals and enzymes and has a harmonizing affect on the body.

Water water water before, during and after.  

The mother should be taking in a minimum of 4oz of fluid every hour.

Yogini Postpartum Herbal Baths


YOGINI POSTPARTUM HERBAL BATHS 

Postpartum Delight – a blend of beautiful and fragrant blossoms that are uplifting, soothing, healing and antiseptic.

2 ounces of comfrey leaves

1 ounce of calendula flowers

1 ounce of lavender flowers

¾ cup of sea salt

Deep Healing Bath – strongly antiseptic and astringent, perfect for healing trauma to the perineum, including tears and episiotomies

1 ounce of comfrey leaves

1 ounce of yarrow blossoms

1 ounce of dried sage leaf

1 ounce of rosemary leaf

3/4 cup sea salt

DIRECTIONS:  Bring 4 quarts of water to a boil.   Turn off heat and place 1 ounce (approx one handful) of the herb mix (not the salt) into the pot.  Steep, covered for 30 minutes.  Strain the liquid well with a fine mesh strainer, and discard the herb material.  Add 2 quarts of liquid to the tub, along with the ¾ cup of salt.  Reserve the remaining liquid for another bath or for compresses or peri-bottle.

 If you don’t want to use loose herbs, you can have a bath with the following essential oils.  Make sure to use ¾ of a cup of Epsom Salts!!

Nourishing the mother throughout pregnancy, and the postpartum period is important you’re your body, mind and spirit.  These delicious green drinks are full of nutrients for you and your growing baby.   A wonderful addition to anyone’s diet to drink up this spring, hydrating, full of vital nutrients and yes, tasty!!  
Easy to make, any blender will do, I have been using a hand-me down blender bought at a garage sale.   It does not have to be expensive to be healthy, and feel good.

GREEN fruit smoothie recipe:

1 frozen banana

1 cup of blueberries or strawberries

2 handfuls of spinach

Juice of one lime (optional)

2 cups of water or coconut water

Blend and enjoy!  If there is extra, it can be stored in a glass jar in the refrigerator for 24-48hrs, although I suggest drinking it up. 

BLUE-GREEN power smoothie:

2 Tablespoons of hemp protein powder

1 Tablespoon of maca root powder

3 Tablespoons of spirulina or other blue-green algae

2 bananas (ripe or frozen)

3 cups of water

BIRTH DOULA SERVICES         
 
A doula is a woman who is trained and experienced in the physical and emotional needs of birthing women and families. She understands the physiology of labour and birth, and how to help a woman and her partner work with the process. Whether the birth takes place in a hospital, birthing center or home – whether the baby is delivered by a doctor or midwife, a doula is a valuable asset to any birth team.  Research shows that the support of a doula throughout pregnancy optimizes a woman’s chance of having a healthier birth experience and  a healthier infant.

Since the beginning of time, women have been supporting women through childbirth and the postpartum period. It has been the collaborative effort and experiences of these women that have encouraged and empowered them to transition into motherhood successfully. In our fast-paced modern medical society, we have forgotten this very essential support system that has proven to help women give birth naturally.
Studies have shown that the presence and support of a professional labor assistant (doula) can:

  • Shorten Labor by 25%
  • Reduce the overall cesarean rate by 50%
  • Decrease the need for pain medication by 30%

It’s never too late to hire a doula. She can join you at any point in the process.

BIRTH DOULA PACKAGE

·         Complimentary, no obligation, introductory meeting to help determine if we are a good match and if you’d like me to be a part of your birth support team.
      ·         Two prenatal visits to assess your needs and desires for birth, define and understand our expectations of one another, and provide you with useful tools for your birthing time.

·         Labour support from the beginning of active labor (or earlier if needed) at home or at the hospital
       ·         Continuous physical, emotional, and informational support for you and your partner for the duration of your birthing time, and up to 2 hours postpartum to help you hold and move towards the vision you created for your baby’s birth.

·         Initial breastfeeding support immediately after birth to help your baby latch on properly and encourage the establishment of breastfeeding.

·         Two postpartum follow-up visit to celebrate the birth of your baby and answer any questions you have during the early postpartum period.  Also, help with basic breastfeeding, baby care and new family adjustment.

My fees is $1200.  The payment is divided up into two payments.  A $300 retainer, and the remainder is in a post-dated check dates for the due date – both are given at the same time.  Post-dates check will be cashed after the birth.

WELCOME

Danielle Denwood is a Certified Professional Midwife, Doula, Interdisciplinary Yoga Teacher, and Chartered Herbalist. Grateful for her life path of educating, nurturing, and creating safe space for women during their journey through pregnancy and birth. It is her intention to support women as they take responsibility for their own health and well-being and that of their baby. It is her hope that birth will be a positive, transformative experience for mothers and families, fostering empowerment throughout their lives.

As a Certified Professional Midwife, Danielle’s next journey is to Uganda to work with Mother Health International for September and October of 2014. Her work as a midwife inspires her deeply, and she believes that education and informed choice are just as important as breath and nourishment for a healthy pregnancy and birth.

Danielle currently teaches Pre-Natal Yoga and Childbirth Education classes throughout the city of Toronto, and is also taking clients as a Doula. She brings a down-to-earth approach, knowledge and warmth to her work as a teacher and birth care provider. Danielle looks forward to opportunities to share with pregnant women the joy, empowerment and sense of community that is created through doing Pre-Natal Yoga together.
